Click on the Membership form to download and follow steps below:
1). Download the PDF Membership Form. Click on the form to fill out all highlighted information. (You can tab from one highlight to another to complete the form.) 2). Once the form is completed, click on FILE at the top left of the screen . Click on SAVE AS. When the SAVE AS box pops up, change the Name of the file by adding your last name at the end (ex: PPWA Membership Fillable Form-Last Name). Then select SAVE . 3). Attach your completed application to an email and send it to: sdolembo@verizon.net. It will be matched to your PayPal payment. If not using PayPal, print your application and mail your check and application to: PPWA Membership 821 Tropez Lane Venice, FL 34292 Please note screenshots of your application are not acceptable. Thanks. iPAD QUESTIONS? ISSUES: PLEASE NOTE: When using an iPad, you must install Google Drive in order to use the fillable PDF membership form. Google Drive is only available on IOS 14.0 and later. To determine what version your IOS device is running, go to Settings: 1. Click on the field with your name, Apple ID, iCloud, Media & Purchases. 2. Scroll down to iPad 3. Under Device Info: • model • version • serial number to the right of version, you will find what operating system is running on your device. PLEASE ALSO NOTE: The database will only work if you are using CHROME as your browser on your iPad. |
